Access to Care Rounds

Eastern Colorado GRECC (Geriatric Research, Education and Clinical Centers)  started Access to Care Rounds as a way to ease the transition for Veterans between the VA and community providers and vice versa. These Access to Care are designed to bring community care providers face to face with medical professionals in VA settings to discuss the needs of the Veteran and develop a treatment plan so no one "slips through the cracks".
