4F-PCC Enables Faster VKA Reversal Before Urgent Orthopedic Surgery
Sunday, 19 July, 2026
Host: Matt Birnholz, MD A post hoc analysis of a randomized phase 3b trial evaluated four-factor prothrombin complex concentrate (4F-PCC) versus plasma for urgent vitamin K antagonist (VKA) reversal in adults requiring orthopedic surgery. While both treatments demonstrated comparable hemostatic efficacy and similar safety profiles, 4F-PCC produced substantially faster INR reduction and shortened the median time to surgery by more than 13 hours compared with plasma. These findings add to the growing body of evidence supporting rapid anticoagulation reversal with 4F-PCC in time-sensitive orthopedic procedures. Tune in as Dr. Matt Birnholz reviews the study results, their clinical implications, and important limitations that should inform our interpretation.
